ALS Ice Bucket Challenge Progress

Medical Information Materials

Over the past several months, The ALS Association has developed a suite of medical information materials intended to intended to educate and inform healthcare professionals and emergency personnel, like paramedics, who may have many questions about how to provide care for someone living with ALS. There are currently three items associated with this toolkit, including:

1. Medical Information Packet

The Medical Information Packet is a collection of individual pages, which provide insurance, family, and healthcare team information, in addition to educating medical professionals about ALS and the considerations one needs to be aware of when treating someone with ALS. This resource may be used as a packet or as individual pages, based one’s preference and need. The packet can be filled out and downloaded from The ALS Association’s website http://www.alsa.org/assets/pdfs/Medical-Information-Packet-ERHospitalInfoPacket.pdf or ordered in hardcopy from the Care Services Order Portal http://portal.alsa.org/.

2. Key Medical Information Card

The Key Medical Information Card is a condensed version of the medical information packet and contains fundamentally important considerations that a medical professional needs to be aware of when treating someone with ALS. The card can be easily folded and is intended to be carried around or easily accessible in the event of an emergency situation. An electronic version of the card is available on The ALS Association’s website http://www.alsa.org/assets/pdfs/Medical-Information-Packet-ZCard.pdf, but can also be ordered in hardcopy format from the Care Services Order Portal http://portal.alsa.org/.

3. Key Medical Information Mobile App

The Key Medical Information app provides medical personnel with critical information to care for a person with ALS during an emergency. In addition to medical dos and don’ts, the app provides a communication board and emergency contact fields for users to reference. The app is available in Google Play and the Apple Store and is searchable by “Key Medical Information” or “The ALS Association”.
